2025-26 ATHLETIC CLEARANCE INFORMATION:

All student-athletes interested in participating in a sport for the 2025-26 school year must have a valid physical on file with the school prior to the first practice date in the fall, spring and winter.  All other clearance forms need to be completed prior to the first contest date each season.

• Physicals are valid for two years after the date of the exam.
• If a physical expires, an updated physical must be submitted before the start of the season.
• Please turn in completed physical forms to Rick Gardner in one of the following ways:

  • Turn in or mail to Marist Catholic High School at 1900 Kingsley Road, Eugene, OR 97401
