2025 ATHLETIC CLEARANCE INFORMATION:

All student-athletes interested in participating in a 2025 fall sport must have a valid physical on file with the school prior to the first practice on Monday, August 18th.  All other clearance forms need to be completed prior to the first contest date on Thursday, August 28th.

• Physicals are valid for two years after the date of the exam.
• If a physical expires, an updated physical must be submitted before the start of the season.
• Please turn in completed physical forms to Rick Gardner in one of the following ways:

  • Turn in or mail to Marist Catholic High School at 1900 Kingsley Road, Eugene, OR 97401
  • Fax to 541-342-6451

Participation Agreements: These online forms must be completed once every school year.
